Board of Education Vacancy Notice Form
Per Illinois School Code (105 ILCS 5/10-10), the Board of Education (BOE) shall notify the Regional Superintendent within five (5) days of the occurrence of any vacancy on the BOE. Please complete the form below to notify North Cook ISC when a Board vacancy occurs.
The BOE has 30 days to fill the vacancy by appointment until the next regular school election. If you have filled the vacancy, please provide the information about the person appointed in the form in Section 3. If you do not yet have that information, please complete Section 1 and 2 within 5 days of the occurrence of the vacancy. Then, upon appointment, email the information about the new Board member to NCISC Executive Director April D. Jordan, Ed.D., ajordan@ncisc.org.

Bus Accident Report Form
Complete a Bus Accident Report Form when reporting a school bus accident that involves your district’s transportation services, whether provided internally or through contractual services.

Clear and Present Danger Determination Form
Pursuant to the Firearms Owners Identification (FOID) Card Act, when a student or other person demonstrates threatening physical or verbal behavior determined to pose a clear and present danger to themselves or others, school administrators must submit a report to the Illinois State Police (ISP) within 24 hours of the incident. This reporting process is intended to identify and prevent individuals who pose a clear and present danger from legally accessing firearms or firearm ammunition by revoking the individual’s FOID Card or preventing them from obtaining a FOID Card in the future. Reports should include specific details, dates and times, as well as the names and contact information of any witnesses.

Complete the top portion of the form and send it with a copy of your district's e-Learning Plan to:

Dr. Jordan will verify that the plan addresses the 12 requirements listed in the body of the form. Once verified, the Executive Director will sign the form and return it to the district Superintendent for final execution. Then, annually, for three years, the Executive Director will verify the plan.

Fire Form
In the event of a fire at a public school outside of Chicago, school district officials should complete the School Fire Report Form as soon as it is reasonable to do so.

Institute Day Applications
You must submit your Teacher Institute (TI) Day application at least 30 days prior to a scheduled Institute Day. A district is allowed up to four TI Days per school calendar year and they must be labeled as "TI" on your Public School Calendar submitted to IWAS.
Make sure your institute day advisory committee complies with the criteria outlined in Illinois School Code 105 ILCS 5/3-11. The advisory committee must be composed of 50% teachers, 25% school administrators, and 25% school service personnel.
In addition:
- The district superintendent must approve all applications before they are submitted.
- The Institute Day agenda must be uploaded to the online application. Five hours of professional development must be clearly outlined and excludes time for registration, meals, snacks or breaks.

Report of Students Removed from School Rolls (105 ILCS 5/26-3a)
Districts must share their unenrollment lists with NCISC quarterly on the first school day of October, January, April and July. This requirement does not pertain to students who have transferred, been expelled or withdrawn from the district. The report includes the names and addresses of students formerly in attendance, the names and addresses of persons having custody or control of the students, the reason (if known) that the students are no longer in attendance and the date of removal from the attendance rolls. The list must also include the names of: students whose withdrawal is due to extraordinary circumstances, including but not limited to economic or medical necessity or family hardship, as determined by the criteria established by the school district; students who have re-enrolled in school since their names were removed from the attendance rolls; any students certified to be a chronic or habitual truant, as defined in Section 26-2a; and students previously certified as chronic or habitual truants who have resumed regular school attendance.
