Fingerprinting
North Cook ISC provides fingerprint-based Criminal History Records Information (CHRI) checks through the Illinois State Police and the FBI for educators. Appointments last for approximately 20-30 minutes. Background checks are usually processed within a week, but in rare cases, results can take as long as eight weeks.
These services are ONLY available to educators that will be working at a North Cook school district. View the list of North Cook Districts.
How to Schedule an Appointment
Click on the BOOK APPOINTMENT NOW button, then follow these three steps:
- Screen 1: Select a type of service
- Screen 2: Select a date and time
- Screen 3: Enter your details – Required information includes your cell phone number, email address, first and last name. Appointment notes are optional, but if you know the name of the school district where you will be working, please list the school district's name and district number in this section.
You will receive a confirmation of your appointment via email and a text message. As stated at the bottom of the the details screen, Square will automatically create an account for you with Square Appointments. You can return to this page, click on the BOOK APPOINTMENT NOW button again, and sign in using your cell phone number to make changes at any time.

Before you book your appointment, complete the fingerprint authorization and release/FBI privacy act form. (You may have already received this form from your school district's HR department.) It is a fillable form so you can complete it on your computer if you have Adobe Reader.

- A valid photo ID such as an IL Driver's License or passport. University or school IDs are not acceptable forms of identification).
- A credit card for payment using Square (the only form of payment that will be accepted. Cost: (If the cost is paid on your behalf by a school district, you must bring a letter/note on district letterhead to the appointment as verification.)
- $35 for student teachers plus $1.04 credit card processing fee
- $50 for contractual or substitute teachers plus $1.44 credit card processing fee
