    FINGERPRINTING SERVICES

    North Cook ISC offers fingerprinting in-person BY APPOINTMENT ONLY. To ensure protection for our staff as well as the educator, we've set up an area in our office that adheres to all social distancing guidelines and necessary safety protocols.

    If you work for a school district and need to have someone fingerprinted right away, please send an email with the name of the individual to Izabela Kuczek at ikuczek@ncisc.org.

    Each candidate must complete the following forms:

    1. FINGERPRINT AUTHORIZATION & RELEASE and FBI PRIVACY ACT STATEMENT FORM

    This 2-page form MUST BE DOWNLOADED, opened, and filled out in Adobe Reader. It can also be saved to your computer. If you need the Adobe Reader application, click on Adobe Acrobat Reader for a free download.

    2.NCISC VISITOR HEALTH PRE-CHECK FOR FINGERPRINT APPOINTMENT FORM

    This form MUST BE DOWNLOADED, opened, and filled out in Adobe Reader. It can also be saved to your computer. Please fill out this form completely and accurately before setting up your fingerprinting appointment. Regardless of how you answer the questions, our staff will work with you to arrange the most appropriate time for you to be fingerprinted. Your answers to these questions are for our internal office use only and will be kept confidential.
     
    Please send the two completed forms to Izabela Kuczek at ikuczek@ncisc.org. Once the forms are received, Izabela will contact the individual educator to schedule an appointment. Before being given an appointment time, the individual must "pass" the questionnaire and agree to wear a mask and maintain appropriate social distancing behaviors while in person at the North Cook ISC office.

    3.  On the day of your scheduled appointment, you must fill out our online Visitor Health Pre-Check for Appointment form which is located on our Contact Us page.
     
    These are unique times and we are attempting to minimize time spent in our office, which requires candidates to be patient and flexible. We have limited time slots and appointments available.

    North Cook ISC provides finger-print-based Criminal History Records Information (CHRI) checks through the Illinois State Police and the FBI. Proof of valid identification is required at the time of printing.

    Please call to schedule an appointment at our office. Appointments are scheduled most Mondays - Fridays, between the hours of 8:00 a.m. and 3:00 p.m.
