Under the Illinois Freedom of Information Act (FOIA), access to NCISC's public records is available to any person upon written request. Written requests for public records may be made via the form below, via e-mail, or may be submitted by personal delivery, mail or fax directed to the NCISC Freedom of Information Officer. Individuals making a request are not required to state a reason for the request other than to identify when the request is for commercial purpose or when requesting a fee waiver.
