Providers of Education to North Cook English Language Learners NCISC is studying how to best automate registration processes in coordination with local district practices. In the future, this form will be filled out on line and sent to NCISC to advise us of your interest in the program and intention to attend. Currently, it can be filled out on line BUT must be printed out and submitted by fax (847-824-1033) or by mail (NCISC, 2340 Des Plaines River Road, Des Plaines, IL 60018). Do this immediately to reserve your place in the course. Purchase order and/or payment must be received prior to the course date. Please Note: Refunds issued only to participants who call the NCISC office seven (7) days before the starting date of the program, or if the program is cancelled due to insufficient enrollment.
